1. Who We Are
Top Rated Physical Therapy & Massage (“we,” “our,” or “us”), formerly Utah Mobile PT, provides mobile physical therapy, therapeutic massage, and vestibular and balance care in Utah and Idaho. You can reach us at (385) 262-3306.
2. Scope of This Policy
This policy covers information collected through this website and our related communications, including text messages.
It does not cover your medical records. Protected health information created or received in the course of treatment is governed separately by our Notice of Privacy Practices under the Health Insurance Portability and Accountability Act (HIPAA). Ask us for a copy if you have not received one.
3. Information We Collect
Information you give us. Our contact form collects your first name, last name, and email address, which are required, and your phone number and a description of how we can help, which are optional. If you check the SMS Consent box, we also record that consent and when you gave it.
Information collected automatically. Like most websites, our host records standard technical information such as IP address, browser type, pages requested, and timestamps. Our contact form is provided by Zoho and is embedded from their servers; when it loads, Zoho receives technical information about your visit and may set cookies, subject to their own privacy policy. Our web fonts are served by Google Fonts, which likewise receives a request from your browser.
We do not run advertising trackers or analytics profiling on this site.
4. How We Use Information
- to respond to your enquiry and schedule care;
- to contact you about your appointment by phone, email, or, with your consent, text;
- to coordinate billing with the applicable insurer;
- to operate, secure, and improve the website;
- to meet our legal, regulatory, and professional obligations.
5. Sharing of Information
We share information only as needed to run our practice: with service providers who work on our behalf, such as our form provider and messaging vendors; with insurers and, where you direct us, your attorney, for billing and claims; within the Injury Accident Network family of companies where necessary to coordinate your care; and where required by law.
We do not sell your personal information. No mobile information, including SMS opt-in data, is shared with third parties or affiliates for their own direct marketing or promotional purposes.
6. Text Messaging (SMS) Program
If you check the SMS Consent box on our contact form, you consent to receive marketing, customer care, and account notification-related messages from us. Message frequency varies. Message and data rates may apply. Reply HELP for help or STOP to cancel at any time. Consent is optional and is never a condition of receiving care. Full details are in section 8 of our Terms of Service. SMS is not a secure channel, so please do not send sensitive medical or payment details by text.
7. Data Security
We use reasonable administrative, technical, and physical safeguards to protect the information we hold. The website is served over HTTPS. No method of transmission or storage is completely secure, so we cannot guarantee absolute security.
8. Data Retention
We keep enquiry information for as long as needed to respond to you and to meet our legal and business obligations, then delete or de-identify it. Medical records are retained under the separate schedules that apply to health records in the states where we practice.
9. Your Choices and Rights
- Ask us what information we hold about you, and ask us to correct it.
- Ask us to delete enquiry information, subject to records we must legally keep.
- Opt out of text messages at any time by replying STOP.
- Ask not to be contacted for marketing purposes.
- Exercise the rights in your medical records through the process in our Notice of Privacy Practices.

10. Children's Privacy
This website is intended for adults aged 18 and over (see section 5 of our Terms of Service) and is not directed to children under 13. We do not knowingly collect their information through it. A parent or legal guardian should contact us on behalf of a minor. If you believe a child has given us information through this site, call us and we will delete it.

Where we link to or embed services we do not control, their privacy policies apply to the information they collect. We encourage you to read them.
12. Changes to This Privacy Policy
We may update this policy. Changes take effect when posted here with a new effective date.
